Kerry Johnson made the plea yesterday while cuddling six-week-old grandson Bradley Lapham, who has been without his mum, Leeann Lapham, for 17 days.
Ms Johnson yesterday broke her silence on her heartache since her daughter, Ms Lapham, 30, vanished without a trace at 7.30pm on April 19.
Ms Lapham was last seen leaving the Innisfail Riverside Motel.
"If Leeann is out there, please, come home, Lee – your son needs you – so do I," a tearful Ms Johnson said at the Innisfail Police Station yesterday.
"I really need you to come home, please. I miss you – Bradley needs you really badly.
"I love you, girl. So much. I don’t care about whatever you are doing, just come home.
"If anybody knows where she is, please, get in touch with the police and let them know."
Ms Johnson described her daughter as beautiful, bubbly and strong.
"It’s a nightmare," she said, as her partner, Frank Catalan, rubbed her shoulder.
Ms Johnson filed a missing persons report with police on April 27.
Police hold fears for Ms Lapham because she has not accessed her bank accounts or used her mobile phone since she went missing.
Coast Guard and SES volunteers have searched parts of the Johnstone River, near the motel where she was staying.
Ms Lapham is described as being 170cm tall with a slim build, straight brown shoulder-length hair, green eyes and fair skin.
She was last seen wearing long black pants, a blue halter-neck top and carrying a large leather bag with light and dark brown diamond pattern.
Ms Lapham also has a tattoo of coloured flowers on her right leg, from ankle to mid-calf, black stars on the left calf and the word "RAVEN" on the back of her left wrist.
She also has a tattoo of a woody woodpecker on her stomach and a jester on her right shoulder.

THE mother of a missing Innisfail woman says she has nightmares about her daughter and has begged for her safe return.
Dressed in zebra-like striped clothing and cuddled by his loving grandma Kerry Johnson, six-week-old Bradley has been without his mum Leeann Lapham for 17 days.

Police yesterday placed a mannequin, wearing clothes the missing woman was last seen in, on a busy footpath in Innisfail.
A Crime Stoppers tent was also erected outside the post office and, within an hour, a witness came forward.
Det-Sgt Dave Tucker of Innisfail CIB said Ms Lapham's disappearance, was ''out of character''.
''With the clothing that she is wearing that just may be the catalyst to prompt someone's memory which will urge them to come forward with information,'' Det-Sgt Tucker said.
''This disappearance is out of character for Leeann given the fact she has a six-week-old child and she had developed a good bond with that child ... that, too, is of great concern that a mother would voluntarily leave her baby.''
