Victorian Police have concerns for a Frankston woman who has been missing since yesterday afternoon.

Jade Young was last seen at an address in Witternberg Avenue, Frankston around 4pm yesterday.

She left in her grey coloured Toyota Yaris, with registration YFB 301.

Jade is reliant on regular medication for a medical condition and her family also hold grave concerns for her welfare.

The 27-year-old is described as Caucasian, has shoulder-length brown hair, a thin build and fair complexion.

THE FAMILY of a Frankston woman who went missing yesterday afternoon hold grave concerns for her welfare.

Jade Young was last seen at an address in Witternberg Avenue, Frankston around 4pm yesterday.

The 27-year-old is reliant on regular medication for a health condition.

She is described as Caucasian, has shoulder-length brown hair, a thin build and fair complexion.

Police are continuing to investigate whether she made contact with anyone overnight.
